Appointment Essentials

When you come for your appointment please remember to 
bring the following:

  • Referral letter from your GP, other medical specialist or physiotherapist
  • Medicare card, DVA card, Health Care Concession Card
  • Private Medical Insurance information
  • Any available reports, X-rays, MRIs, CT scans
  • List of current medications

If you have not had a recent x-ray, CT, or MRI please let us know as we may ask that you have one on arrival to our clinic. If you have any imaging performed, please note which radiology provider performed this so we can ensure we have access to your scans at the time of your appointment.

As part of your consultation, Dr Calvert will perform a physical examination. It is important to dress in a way that allows for examination of the problematic areas. We recommend you wear loose, comfortable clothing to your appointment.

Dr Calvert is a No Gap Provider for HBF patients and has agreed gaps for other insurers.

Before your operation

To ensure the best care possible Dr Calvert will need to know your medical history and current medications.

Please inform him if you have a history of:

  • Bleeding disorders or are currently taking a blood thinning medication
  • Deep vein thrombosis or pulmonary embolism 
(blood clots)
  •  Heart or lung issues
  •  Diabetes (type 1 or type 2)
  • Joint infection (current or previous)
  • Smoking

It is important to remain as active as possible in the lead up to surgery to improve your recovery.
